I was talking with another VA the other day, and she said I really should have a client intake sheet. Do any of you use one? If so, what sort of information is on yours?

I do have a pre-consult questionnaire I use to help me decide if the prospect is even someone I want to work with. It has all the information I'd then need once then become a client. I've attached mine for your review.

Yes, I use a client intake sheet. It's a tidy way to list your services and to learn more about potential clients. Make sure to include a segment where the client can write down any questions so that when you and the client meet in person or online, you have a beginning checklist.

My intake sheet includes the name of my company, contact info, and website. It also lists my services in checklist format with a couple blank lines for other;

I ask the following: best times of day for me to contact the client; best days of the week for me to contact the client; client - name, organization, address, telephones, email addresses, and website.

When the client provides a website or company information, I perform a quickie Google search to make myself more knowledgable about the client and this is a great opening for our initial conversations.
