I just had a client let me know if I sent them a testimonial request form, they would be happy to complete it. Since this is a new client, I hadn't broached the testimonial subject yet -- they brought it up.

Does anyone use a formal form to request a testimonial from a client? Or, do you just ask for a testimonial and receive it? I've always just asked.

Never heard of anyone using a form. Usually I just use comments clients send in an email or by written correspondence and ask for the client's okay to use what they've already said as a testimonial.

If you have anything in writing already from them, perhaps you can tweak it and write the testimonial, then ask them if it meets with their approval, if you can then post it in your marketing materials and on your website.
After working with them for awhile and getting positive feedback I request a testimonial from my clients. They have been happy to write it and provide it to me to use as I wish.

I think a form would be too stuffy and might turn off clients from giving the testimonial :)
Sue and Cindy, thanks for your response. I agree, I would normally just ask a client after they became more established. This new client brought up the subject after our first project. Unfortunately, they only e-mail communication (or communication in writing) has been project delivery on my end. The client likes to call with input and further instruction.

Since this particular client asked for a testimonial form (and I personally have never used one), I was wondering if anyone else might use them. Again, thanks for sharing your thoughts.
Another thing you might do since they like to use the phone is ask their permission to record their audio testimonial and then transcribe it. If you do decide to use a form, I'd suggest something very simple and just ask a few questions.

1. Name, Company Name, Link to Website
2. What did you enjoy most about working with Leigh Anne?
3. Would you recommend Leigh Anne to other business owners?

Hope that helps! Sue

Dear Customer,

Thank you for your business. Happy clients are vital to the success of my business as are
repeat business and referrals. I would appreciate it if you could spare just a few moments to
write a testimonial on the services that we provided and your experience including what you
liked most about us.

To help you with this request, the following questions may prompt some thoughts for you:

•What would you say is unique about me, my services or my way of doing business?

•In what way did I add value to the service for which I was engaged?

•In what ways did I make it easy for you to do business with me?

•Would you recommend me to others?

•Should the opportunity arise, would you use my services again?

I am also using this as a means of evaluating my customer satisfaction levels and would value
any other comments you may wish to add, either as a part of your referral letter or separately
to me personally.

Please call me should you have any questions or concerns about my request.
I sincerely appreciate your help and look forward to our relationship continuing well into the
future.

Kindest Regards,
This is awesome. Thanks for putting such thought into it. I was just wondering if anyone actually used a form to request a recommendation, and you far exceeded what I expected.
