Never heard of anyone using a form. Usually I just use comments clients send in an email or by written correspondence and ask for the client's okay to use what they've already said as a testimonial.

If you have anything in writing already from them, perhaps you can tweak it and write the testimonial, then ask them if it meets with their approval, if you can then post it in your marketing materials and on your website.

After working with them for awhile and getting positive feedback I request a testimonial from my clients. They have been happy to write it and provide it to me to use as I wish.

I think a form would be too stuffy and might turn off clients from giving the testimonial :)

Sue and Cindy, thanks for your response. I agree, I would normally just ask a client after they became more established. This new client brought up the subject after our first project. Unfortunately, they only e-mail communication (or communication in writing) has been project delivery on my end. The client likes to call with input and further instruction.

Since this particular client asked for a testimonial form (and I personally have never used one), I was wondering if anyone else might use them. Again, thanks for sharing your thoughts.

Another thing you might do since they like to use the phone is ask their permission to record their audio testimonial and then transcribe it. If you do decide to use a form, I'd suggest something very simple and just ask a few questions.

1. Name, Company Name, Link to Website
2. What did you enjoy most about working with Leigh Anne?
3. Would you recommend Leigh Anne to other business owners?
